DSIP (Delta sleep-inducing peptide) Description: Delta sleep-inducing peptide, abbreviated DSIP, is a neuropeptide that when infused into the mesodiencephalic ventricle of recipient rabbits induces spindle and delta EEG activity and reducedmotor activities.Delta sleep-inducing peptide was first discovered in 1974 by the Swiss Schoenenberger-Monnier group who isolated it from the cerebral venous blood of rabbits in an induced state of sleep. It was primarily believed to be involved in sleep regulation due to its apparent ability to induce slow-wave sleep in rabbits, but studies on the subject have been contradictory. DSIP is an amphiphilic peptide of molecular weight 850 daltons with the amino acid motif:N-Trp-Ala-Gly-Gly-Asp-Ala-Ser-Gly-Glu-CIt has been found in both free and bound forms in the hypothalamus, limbic system and pituitary as wellas various peripheral organs, tissues and body fluids. In the pituitary it co-localises with many peptide and non-peptide mediators such as corticotropin-like intermediate peptide (CLIP), adrenocorticotrophic hormone (ACTH), melanocyte-stimulating hormone (MSH), thyroid-stimulating hormone (TSH) and melanin concentrating hormone (MCH). It is abundant in the gut secretory cells and in the pancreas where it co-localises with glucagon.In the brain its action may be mediated by NMDA receptors. In another study Delta sleep-inducing peptide stimulated Acetyltransferase activity through α1 receptors in rats.It is unknown where DSIP is synthesized. 4. BACKGROUND AND OBJECTIVE: Delta sleep-inducing peptide (DSIP) is an endogenous peptide that crosses the blood-brain barrier, named after its association with natural sleep and enhanced electroencephalogram (EEG) delta rhythm. The objective of this study was to determine whether DSIP could be used as an adjunct to volatile anaesthesia in humans, our hypothesis being that DSIP is a natural hypnotic that would increase anaesthetic depth. The aims were to assess depth of anaesthesia using bispectral index (BIS), the EEG and heart rate variability (HRV), and to determine whether DSIP altered the symmetry of EEG between the left and right cerebral hemispheres.METHODS:Twenty-four female ASA I or II patients gave written, informed consent to a protocol approved by our local research ethics committee. Twelve were randomly assigned as controls to receive saline. The other 12 were randomly allocated to receive one of three intravenous bolus doses of DSIP (Clinalfa) at 25, 50 or 100 nmol kg(-1). The first administration of DSIP was while awake and the second after induction of anaesthesia with propofol and maintenance with isoflurane. BIS and EEG parameters were measured continuously using a bilateral electrode montage.RESULTS:DSIP significantly increased heart rate, decreased HRV and, paradoxically, significantly reduced delta rhythm along with reducing burst suppression and increasing BIS at 25 nmol kg(-1) during isoflurane anaesthesia. DSIP also significantly altered bilateral symmetry of EEG.CONCLUSION:DSIP probably reduced parasympathetic tone and decreased (lightened) the depth of anaesthesia measured using BIS. 5.
